I'll need you claim information in order to proceed with service. I bill your insurance provider. You do not pay at time of service.

Cancellation Policy

Please cancel as soon as you know you have a time conflict. I prefer a 48+ hours notice--that allows me to free up the appointment and increases the likelihood that someone will scoop it up. IF you cancel in under 24 hours, I charge half the value of your appointment.
